Do you have a chlorine/ chloramine filter on your RO/DI?
what rodi system are you using? How old are the filters?
Filters are pretty new ...just a regular carbon filter
Your tank looks "off" because the beneficial bacteria is no longer around. Remove fish into safe water as their gills are damaged.
if your RO filter is letting chlorine through either the filter is bad or it is missing an important stage to remove chlorine. do you have a tds meter? it should read 0ppm after the DI resin cartridge.

Recommend doubling up on carbon filters before the RO membrane- it’s worth spending extra on at least one to be “universal” or rated for chloramines. Should be replaced every 6mos. Any chlorine past the DI is not good (I may be wrong but I don’t believe chloramines show on tds meter) also may consider replacing the RO membrane. Technically all the filtration before the membrane is done to keep it clean and prolong its life.
I would check your phosphates as well, if the chlorine passed, I would guess phosphates are high as well since it is common in city water. I was having problems about 18 months ago and just started buying RO water instead at 0.30/gallon I was tired of filters and just bring it home for mixing. No problems since.
