Can you get an image from another angle? Also a little bit more deets e.g. Is that a bubble tip? This coming out it's mouth?
It could just be pooping lol... if it's a bubbletip then those nems go through various cycles and I wouldn't be too worried... pull out that funk if it secreted it and just let the nem do its thing

Yeah sorry to say but if that's it's foot then it's probably a goner. If it's deteriorating that bad it's most likely past the point of no return so I agree about a wc and I'd run some fresh carbon as well.
What're the tank perams and how long have you had the nem? Anything happen that might've lead to this?

LTA,(which do not split like BTA) do you have a sand bed for it?
