For clarification are you asking the tip of the tentacle or the mouth???
if your asking about the mouth is swollen and like a ballon and the whole body has retracted it pooping the same way food came in is were its going out.

if your asking about the tip of the tentacle its were they hide the cord and the harpoon for the stingger thats why it have a ballon but most of them loose it in captivity due to no predetor and being hand fed

if it's a bta (bulb tipped anemone) then that's normal...i'd just watch it for a few days...

no one can really prove what causes the tips to bubble, some say light, some say flow, some say diet

Just be happy he's alive:D
